How do I login to WHM after being blocked by cPHulk (root user)?

To gain access to your dedicated server/VPS WHM control panel after multiple login failures (where cPHulk is enabled):

 

Load https://<serverip>:2087/scripts2/doautofixer?autofix=disable_cphulkd in your browser, once logged in you will be able to disable cPHulk.
